The Calcium-Sensing Receptor (CaSR), encoded by the CASR gene, is a G-protein-coupled receptor (GPCR) that plays a pivotal role in maintaining systemic calcium homeostasis. While traditionally studied in the parathyroid gland and kidneys, CaSR is increasingly recognized for its critical functions in the central nervous system (CNS), where it is expressed in neurons, astrocytes, and oligodendrocytes. In the brain, CaSR modulates synaptic transmission, neuronal excitability, and neuroinflammatory responses. It responds to extracellular calcium fluctuations and other polyvalent cations, influencing intracellular signaling cascades that regulate neurotransmitter release, glial activation, and cell survival. Dysregulation of CaSR signaling has been implicated in several neurodegenerative diseases, including Alzheimer’s disease, Parkinson’s disease, and amyotrophic lateral sclerosis (ALS). Notably, CaSR activation has been linked to increased production of amyloid-beta and tau hyperphosphorylation—key pathological features of Alzheimer’s disease. Additionally, CaSR-mediated signaling may exacerbate neuroinflammation and oxidative stress, contributing to neuronal damage and synaptic loss. Given its dual role in calcium sensing and neuroimmune modulation, CaSR represents a promising therapeutic target for neurodegenerative disease intervention. Pharmacological modulation of CaSR activity—using calcimimetics or calcilytics—offers potential strategies to restore calcium balance and mitigate neurodegenerative progression.
