The vesicular acetylcholine transporter (VAChT), encoded by the SLC18A3 gene, is an essential membrane protein responsible for packaging acetylcholine (ACh) into synaptic vesicles for release at cholinergic synapses. By regulating the availability of ACh in the synaptic cleft, VAChT plays a central role in modulating cognitive function, motor control, and autonomic nervous system activity. In neurodegenerative diseases such as Alzheimer’s disease (AD), Parkinson’s disease (PD), and amyotrophic lateral sclerosis (ALS), cholinergic dysfunction is a well-established pathological feature. Reduced VAChT expression or impaired transporter function has been linked to synaptic failure, cognitive decline, and neuroinflammation. In AD, for example, VAChT levels correlate with disease severity and are considered a potential biomarker for cholinergic integrity. Recent advances in molecular imaging and transgenic models have enabled precise tracking of VAChT expression in vivo, offering new insights into the progression of neurodegenerative disorders.
