Aquaporin 1 (AQP1) is a widely expressed integral membrane protein that functions as a selective water channel, facilitating rapid water transport across cell membranes while excluding ions and solutes. Traditionally studied in renal physiology—where it is localized to the proximal tubules, descending loop of Henle, and vasa recta—AQP1 is also found in red blood cells, vascular endothelium, lungs, gastrointestinal tract, and sweat glands. Unlike Aquaporin 2, AQP1 is not regulated by vasopressin. In the central nervous system, AQP1 is expressed in the choroid plexus and certain neuronal and glial populations, where it contributes to cerebrospinal fluid (CSF) production and water homeostasis. Disruption of AQP1 expression or function has been implicated in altered brain water balance, edema, and neuroinflammatory responses—key features of various neurodegenerative conditions. Emerging evidence suggests that AQP1 may play a role in the pathophysiology of Alzheimer’s disease, traumatic brain injury, and multiple sclerosis, where dysregulated water transport and blood-brain barrier integrity are critical factors. Its expression in vascular endothelium also links AQP1 to neurovascular coupling and cerebral perfusion, processes that are often compromised in aging and neurodegeneration. As interest grows in the role of fluid dynamics in brain health, AQP1 is gaining recognition as a potential biomarker and therapeutic target in neurodegenerative disease research.
