Ferroptosis is an iron-dependent, regulated form of cell death driven by the unchecked peroxidation of polyunsaturated phospholipids in cellular membranes, biochemically and morphologically distinct from apoptosis and necroptosis. The central defense against ferroptosis is the selenoenzyme GPX4, a glutathione-dependent peroxidase that reduces lipid hydroperoxides to nontoxic alcohols; its inactivation or loss permits lethal accumulation of oxidized lipids. GPX4 depends on a supply of reduced glutathione, the synthesis of which requires cystine import through the cystine-glutamate antiporter SLC7A11/xCT, making this transporter a key node controlling ferroptosis sensitivity. The execution of ferroptosis also depends on the lipid composition of membranes: ACSL4 preferentially incorporates polyunsaturated fatty acids into phospholipids, generating the substrates that are most prone to peroxidation. A parallel, glutathione-independent surveillance system is provided by FSP1/AIFM2, which regenerates reduced ubiquinone at the membrane to trap lipid radicals. Iron availability is decisive because labile iron catalyzes radical chemistry; this pool is buffered by Ferritin (FTH1), supplied by the Transferrin Receptor, and liberated through ferritinophagy mediated by the selective cargo receptor NCOA4. The transcription factor Nrf2/NFE2L2 governs many antiferroptotic genes, including those for glutathione synthesis and iron handling, integrating ferroptosis with the broader oxidative stress response. Ferroptosis has emerged as a pivotal process in neurodegeneration, ischemia-reperfusion injury, and cancer, where its deliberate induction is being explored to eliminate therapy-resistant tumor cells.
