What is NEFL?

Neurofilament Light Chain (NEFL or Nfl) are cytoskeleton proteins which are made up of 3 main subunits (heavy, middle and light chain). NEFL is located in the peripheral and central nervous system and it plays a role in the determination of axonal caliber and the growth of axons. This means that NEFL determines the axonal diameter and hence, the conduction velocity of peripheral nerves.

NEFL as a biomarker

When axons are damaged or die, NEFL leaks out of CSF into blood. This means that NEFL concentrations should be higher in neurodegenerative disorders such as Multiple Sclerosis, Alzheimer's disease, Amyotrophic Lateral Sclerosis, Parkinson’s disease and Charcot-Marie Tooth disease. The listed disorders, among others, are debilitating conditions which affect the ability of people to perform everyday tasks such as walking, talking, breathing, swallowing and much more. At present, there are treatments to help slow the progression of neurodegenerative disorders but there are no cures for the diseases.

NEFL has been shown to be a promising biomarker for neurodegenerative disorders. Studies have shown that NEFL levels can provide information on the stage of the disease and it’s progression. As well as this, a treatment response can be monitored through analyzing the levels of NEFL present in blood. When NEFL was analyzed in patients with Alzheimer’s disease, it was concluded that the neurofilament could be a potential screening tool between CSF biomarkers and that elevated levels of the neurofilament light chain are linked to the rate of hippocampal atrophy in cognitively healthy people, including at risk individuals.
