Notch signaling is a juxtacrine pathway that governs cell-fate decisions, differentiation, and stem-cell maintenance through direct contact between neighboring cells. Signaling is initiated when a transmembrane ligand on one cell, such as Jagged2 / JAG2 or DLL4, engages a Notch receptor on an adjacent cell. The Notch receptor family includes Notch1, Notch2, and Notch3, each of which is processed and activated through a conserved mechanism. Ligand binding triggers successive proteolytic cleavages of the receptor; the final intramembrane cleavage is carried out by the gamma-secretase complex, whose catalytic core is Presenilin1, releasing the Notch intracellular domain. This domain translocates to the nucleus and converts the CSL transcription factor from a repressor into an activator, inducing target genes of the HES and HEY families, including Hes1 and Hey1, which act as transcriptional repressors to enforce the chosen cell fate. The pathway is highly context dependent, functioning as either an oncogene or a tumor suppressor depending on tissue and signal strength. Activating Notch1 mutations are a hallmark of T-cell acute lymphoblastic leukemia, while Notch activity supports stemness, angiogenesis through DLL4, and therapy resistance in solid tumors; conversely, Notch can suppress growth in certain squamous and skin cancers. The dependence of many tumors on gamma-secretase-mediated activation has driven development of gamma-secretase inhibitors and ligand- or receptor-targeted antibodies. Profiling receptor and ligand expression, Presenilin1 levels, and induction of Hes1 and Hey1 reports on pathway engagement and activity.
