The tumor suppressor p53 is a central hub that integrates diverse stress signals, including DNA damage, oncogene activation, and nucleotide depletion, into transcriptional programs that enforce cell-cycle arrest, senescence, apoptosis, and metabolic adaptation. Under unstressed conditions, p53 levels are kept low by the E3 ubiquitin ligase MDM2, which binds the p53 transactivation domain, blocks its activity, and targets it for proteasomal degradation in a tightly controlled negative feedback loop. Genotoxic stress activates kinases of the DNA damage response that phosphorylate p53, notably at serine 15 detected as Phospho-p53 S15, disrupting the MDM2 interaction and leading to p53 stabilization and accumulation. Activated p53 then induces an extensive set of target genes. Transcription of the CDK inhibitor p21 imposes cell-cycle arrest, while induction of the pro-apoptotic BH3-only proteins PUMA and NOXA tips the mitochondrial balance toward apoptosis. The stress-inducible factor GADD45 contributes to cell-cycle checkpoints and DNA repair. The p53 family member p63 governs epithelial development and stemness and modulates overlapping transcriptional outputs. In cancer, the p53 pathway is inactivated in the majority of tumors, either through direct TP53 mutation or through amplification of MDM2 and loss of upstream regulators, allowing damaged and oncogene-bearing cells to evade arrest and death. Restoring p53 function or disrupting the MDM2-p53 interaction with small-molecule antagonists is an actively pursued therapeutic strategy. Monitoring p53 abundance and phosphorylation, MDM2 levels, and induction of p21, PUMA, NOXA, and GADD45 reports directly on pathway integrity and activity.
