T-cell exhaustion is a distinct differentiation state that arises during chronic antigen exposure, as occurs in persistent viral infection and cancer, in which T cells progressively lose effector function, proliferative capacity, and cytokine production. This dysfunctional state is enforced by the sustained expression of inhibitory immune checkpoint receptors that counterbalance activating signals. PD-1 is the cardinal exhaustion marker; engagement by its ligand PD-L1, frequently upregulated on tumor cells and antigen-presenting cells, recruits phosphatases that dampen proximal TCR signaling. CTLA-4 acts earlier, outcompeting the costimulatory receptor CD28 for B7 ligands and raising the threshold for activation, while functioning prominently in regulatory T-cell-mediated suppression. Additional coinhibitory receptors define the depth of exhaustion and offer complementary therapeutic targets: VISTA/VSIR is a negative regulator highly expressed on myeloid cells and T cells, TIM-3 marks terminally exhausted cells and binds galectin-9 and other ligands, and CD96 competes with activating receptors for nectin ligands to restrain NK- and T-cell responses. The fate and reversibility of exhausted populations are governed by transcription factors. TCF1/TCF7 marks a stem-like, self-renewing progenitor pool that sustains the response and mediates the proliferative burst following checkpoint blockade, whereas the T-box factor EOMES is associated with the transition toward terminal differentiation and effector cytotoxic programs. Understanding the interplay between surface checkpoints and these transcriptional regulators is fundamental to cancer immunotherapy and to predicting and improving responses to checkpoint inhibitors. Characterizing these populations demands reagents that resolve both surface receptors and lineage-defining transcription factors.
