Tumor angiogenesis, the formation of new blood vessels from existing vasculature, supplies oxygen and nutrients that allow tumors to grow beyond diffusion limits and provides routes for metastatic dissemination. The dominant driver is the vascular endothelial growth factor system. The secreted ligand VEGFA, often induced by hypoxia, signals primarily through the receptor VEGFR2 on endothelial cells to stimulate proliferation, migration, survival, and vascular permeability, while the related receptor VEGFR1 / FLT1 acts in part as a decoy and modulates VEGFA availability. Endothelial cell identity within nascent vessels is reflected by junctional CD31/PECAM1, a widely used marker of vascular density. Vessel maturation and remodeling are governed by the angiopoietin-Tie axis: Angiopoietin-2 acts in a context-dependent manner through the endothelial receptor Tie2 to destabilize vessels and prime them for sprouting or regression depending on the local VEGF environment. Mural cell recruitment and pericyte coverage that stabilize new vessels depend on PDGFRB signaling, while the TGF-beta co-receptor Endoglin/CD105 is highly expressed on proliferating tumor endothelium and supports vascular development. The resulting tumor vasculature is typically disorganized, leaky, and poorly perfused, contributing to hypoxia, impaired drug delivery, and selection for aggressive phenotypes. Because angiogenesis is rate-limiting for tumor expansion, the VEGF axis has been targeted clinically with anti-VEGF antibodies, decoy receptors, and VEGFR tyrosine kinase inhibitors, though adaptive resistance and vessel normalization dynamics complicate outcomes. Profiling ligand and receptor expression, endothelial and mural markers, and vessel-remodeling regulators provides a comprehensive readout of the angiogenic state.
