Progression through the G1 phase and commitment to DNA replication at the G1/S transition is the principal point at which mitogenic signals are integrated into a decision to divide. Growth factor signaling induces Cyclin D1, which partners with the cyclin-dependent kinases CDK4 and CDK6 to form active complexes that initiate phosphorylation of the retinoblastoma tumor suppressor. Hypophosphorylated Rb binds and restrains E2F transcription factors; as Rb becomes progressively phosphorylated, generating the Phospho-Rb form, E2F is released to transactivate genes required for S-phase entry, including Cyclin E1. Cyclin E1 then activates CDK2 to complete Rb inactivation and trigger the onset of DNA synthesis, establishing a self-reinforcing switch that drives irreversible commitment to the cycle. This machinery is held in check by the CDK inhibitors p21 and p27, which bind cyclin-CDK complexes to enforce arrest in response to antimitogenic signals, DNA damage, and differentiation cues. The G1/S control network is almost universally disrupted in cancer. Amplification or overexpression of Cyclin D1, activating lesions in CDK4 and CDK6, inactivation of Rb, and loss of the inhibitors p21 and p27 each uncouple proliferation from external control. The dependence of many tumors on CDK4/6 activity has yielded selective CDK4/6 inhibitors that restore Rb-mediated restraint and are now standard in subsets of breast cancer. Quantifying cyclin levels, CDK abundance, Rb phosphorylation status, and inhibitor expression provides a direct measure of where cells sit relative to the restriction point and how they respond to cell-cycle-targeted therapy.
