Epithelial-to-mesenchymal transition is a reversible developmental program co-opted by carcinoma cells to acquire the migratory and invasive properties required for metastasis. During EMT, polarized epithelial cells lose cell-cell adhesion and apical-basal polarity and adopt a motile, mesenchymal phenotype. A defining molecular event is the loss of E-Cadherin, the core component of adherens junctions whose repression dismantles epithelial integrity, accompanied by a reciprocal gain of N-Cadherin that promotes a more invasive, migratory adhesion profile in a switch widely used to score the transition. The intermediate filament Vimentin is upregulated as cells acquire mesenchymal architecture and serves as a robust marker of the converted state. This reprogramming is driven by master transcription factors that act largely as transcriptional repressors of E-Cadherin: Snail and Slug of the SNAIL family, the zinc-finger factor ZEB1, and the basic helix-loop-helix factor Twist, which together coordinate downregulation of epithelial genes and induction of mesenchymal and stemness programs. EMT is also tightly linked to Wnt signaling, as nuclear beta-Catenin both reflects loss of E-Cadherin-bound junctional pools and drives transcription that reinforces the mesenchymal, invasive program. In cancer, EMT contributes not only to local invasion and dissemination but also to therapy resistance, immune evasion, and acquisition of stem-like traits, while the reverse mesenchymal-to-epithelial transition supports colonization at metastatic sites. Profiling cadherin switching, Vimentin induction, and the expression of the core EMT transcription factors provides a direct readout of where tumor cells lie along this phenotypic spectrum.
