Cells adapt to low oxygen through the hypoxia-inducible factor pathway, a transcriptional program that reprograms metabolism, angiogenesis, and survival, and that is co-opted in cancer, ischemic disease, and chronic inflammation. The activity of HIF-alpha subunits is controlled primarily at the level of protein stability by oxygen-dependent hydroxylases. Under normoxia, prolyl hydroxylases such as PHD2/EGLN1 use oxygen and 2-oxoglutarate to hydroxylate conserved proline residues on HIF-alpha, creating a recognition motif for the VHL tumor suppressor, the substrate adaptor of an E3 ubiquitin ligase that targets HIF-alpha for proteasomal degradation. A second oxygen sensor, the asparaginyl hydroxylase FIH1/HIF1AN, hydroxylates a residue in the transactivation domain to block coactivator recruitment, providing a graded layer of control. When oxygen is scarce, these hydroxylases are inhibited, HIF-alpha subunits including HIF-2alpha/EPAS1 stabilize, dimerize with their partner, and activate target genes. The resulting transcriptional response promotes anaerobic glycolysis through induction of the glucose transporter GLUT1/SLC2A1 and lactate dehydrogenase LDHA, supports pH regulation via the membrane carbonic anhydrase CA9, and drives angiogenesis through the potent secreted factor VEGFA. This oxygen-tuned stability switch allows the pathway to respond rapidly and proportionately to falling oxygen tension across diverse tissues. In tumors, loss of VHL function or intratumoral hypoxia leads to constitutive HIF activity that fuels metabolic adaptation, vascularization, and aggressive behavior, making this axis an important therapeutic target. Examining FIH1/HIF1AN, HIF-2alpha/EPAS1, VHL, PHD2/EGLN1, CA9, GLUT1/SLC2A1, VEGFA, and LDHA together captures both the oxygen sensing apparatus and the metabolic and angiogenic output it controls.
