Cells continuously generate reactive oxygen species as byproducts of respiration and signaling, and must counter them to prevent oxidative damage to lipids, proteins, and DNA. The master regulator of the antioxidant defense program is the transcription factor Nrf2/NFE2L2, which controls a battery of cytoprotective genes through antioxidant response elements in their promoters. Under basal conditions Nrf2 is held in check by KEAP1, a substrate adaptor for a Cullin3 ubiquitin ligase that targets Nrf2 for continuous proteasomal degradation. KEAP1 bears reactive cysteine residues that act as redox sensors; when modified by oxidants or electrophiles, KEAP1 can no longer promote Nrf2 turnover, allowing newly synthesized Nrf2 to accumulate, translocate to the nucleus, and activate transcription. Among its many targets are HO-1/HMOX1, which catabolizes heme to yield antioxidant and anti-inflammatory products; NQO1, which detoxifies quinones and maintains redox cofactors; and the lipid hydroperoxidase GPX4, which protects membranes from peroxidation. The response also encompasses the superoxide dismutases SOD1 in the cytosol and SOD2 in the mitochondrial matrix, which convert superoxide to hydrogen peroxide, and catalase, which decomposes hydrogen peroxide to water. This network is central to redox homeostasis, and its dysregulation contributes to neurodegeneration, chronic inflammation, cardiovascular disease, and cancer, where constitutive Nrf2 activation can paradoxically promote tumor survival and chemoresistance. Examining Nrf2/NFE2L2 and KEAP1 alongside HO-1, NQO1, SOD1, SOD2, catalase, and GPX4 enables a comprehensive readout of both the regulatory switch and its downstream effectors.
